xAPI forms

Overview

xAPI forms allow the user to create customisable checklists which can form part of an assessment or be used as inductions. xAPI forms created on the desktop version of Glasshouse can be synced to the mobile app for on-site inductions or competency checks. Data added to the form in the mobile app can then be synced to appear in the desktop version of Glasshouse.

How to create an xAPI form

Creating the xAPI form

Start in the Page management area of the Sequence page.

  1. Click on the (Select type to create) dropdown menu above the Blocks area, and select Form block.
  2. Click Add.
  3. Enter a block heading and content in the Main tab as required.
  4. Click Save and Publish.

Checklist

Navigate to the Form Elements tab

  1. Click on the (Select type to create) dropdown menu and select Form Checklist. Click Add.
  2. Add a title in the Title input field - this will appear in the back-end only for navigation purposes.
  3. Click Save. Once saved, the default checklist choices Yes and No will appear in the Choices section.
  4. To customise these default options, click on the choice (e.g. Yes) and type the checklist option into the Option field. Click Save and close

The way in which the checklist is displayed can be customised by selecting the How is the checklist going to be displayed dropdown menu. Display options include Checklist, Toggle and Horizontal buttons. If Toggle is selected, the text for each choice will be replaced by a toggle button. Both the Checklist and Horizontal buttons styles show the choices options.

To exit the form element click Save and close.

Button

Start in the Form Elements tab

  1. Click on the (Select type to create) dropdown menu and select Form Button. Click Add.
  2. Add a title in the Title input field - this will appear in the back-end only for navigation purposes.
  3. Enter text into the Label for this form element input field. This is the text that will appear on the button. If this is left blank the button label will default to Submit.
  4. Click Save and close.

Content

The content option allows a section of text to be added within the form independently of individual form elements. This can be useful if instructions or plain text need to be added.

Start in the Form Elements tab

  1. Click on the (Select type to create) dropdown menu and select Form Content. Click Add.
  2. Add a title in the Title input field - this will appear in the back-end only for navigation purposes.
  3. Add text into the Content input field. This will appear as plain text within the form.
  4. Click Save and close.

Text

Start in the Form Elements tab

  1. Click on the (Select type to create) dropdown menu and select Form Text. Click Add.
  2. Add a title into the Title input field - this will appear in the back-end only for navigation purposes.
  3. Type any instructions for the text box into the Text box label input field. This will appear above the text box in the form.
  4. Add text to the Save button label input field. This text will appear on a button underneath the form. 
  5. Add text to the Post Save button label input field. The text on the save button will change to this after it has been clicked and the save process has been successful.
  6. Click Save and close.
Text form options:
  • Click on the Advanced - Past entries accordion under the Save button label input field to view commenting options. Checking the Show past entries checkbox will show all previously entered text as comments once the Save button has been clicked.
  • Click on the Advanced - Default text accordion to add default text which will appear within the text box.
Text Multi

The text multi-option allows you to add a series of text questions to a single block and these questions will only have one save button.

Start in the Form Elements tab.

  1. Click on the (select type to create) dropdown menu, select Form Text Multi and click Add.
  2. Add a title to the Title input field - it will appear in the back end only for navigation purposes.
  3. In the Form Text tab, add Form Text.
  4. Add text to the Save Button Label Input Field. This text will appear on a button underneath the form.
  5. Click Save and close.
Options

File Upload

The file upload option is a mobile-only feature. This allows files to be added from mobile into an existing form which can then be viewed in the web version.
